Programme Specific Requirements for ARCP

In advance of your ARCP you will have been contacted by the Deanery by email in relation to the evidence you are required to submit prior to the ARCP.  All evidence must be submitted at least 2 weeks before the ARCP date.

Please select your programme from the list below and you will find a list of the programme-specific requirements.  If your programme does not appear below, please contact Rosie Baillie.

These requirements are based on trainees working full-time.  If you are LTFT, please discuss with your educational supervisor / TPD and agree your pro-rata requirements.

Requirements for OOP Trainees

Guidance on completing ePortfolio evidence in relation to maintaining patient confidentiality is available from the Academy of Medical Royal Colleges. Please read this before completing your evidence.



Anaesthetics

Please provide the following:

1. Up to date and signed Educational Supervisor`s Report.

2. Annual plus Cumulative Log Books and details of clinical experience and rotations.

3. Completed CUT (Completion of Unit of Training) Forms.

4. Evidence of continued development in relation to Annex G of the Curriculum (Teaching and Training, Academic and Research (including audit), Quality Improvement and Management).

5. Multi-Source Feedback (minimum annually).

6. Consultant feedback.

7. Absence declaration. 

8. SOAR declaration.

9. Exam attempts/Progress.

10. Trainee feedback/evaluation forms.

 

Emergency Medicine

1. Please upload a current CV to e-portfolio which incorporates:

a) list of exams you have passed

b) list of audits and/or quality improvement projects

c) teaching

d) research / publications / presentations

e) list of training days that you have attended/study leave that you have taken during the past year.

2. Please complete the appropriate checklist on e-portfolio for your training level via the RCEM WPBA Schedule.

 

Intensive Care Medicine

1. For detailed evidence list, please see the Faculty of Intensive Care Medicine's Assessment System which includes ARCP decision aids (pp26-28).

2. Please also provide an educational supervisor report that takes into account feedback from trainers who have worked with you.

3. Please provide consultant feedback.

Chemical Pathology

Please provide the following:

1. A current updated CV;

2. Educational Supervisor's Report;

3. Evidence of Workplace based assessments relevant to your stage in training.  Please visit www.rcpath.org (and also www.jrcptb.org.uk if you are a dual trainee doing Metabolic Medicine)

4. Evidence of clinic numbers;

5. College form (indicating examination dates etc). [Form is in process of being finalised and will be available on www.rcpath.org]

 

Clinical Radiology

Please provide the following information:

1. Educational Supervisor's Structured Report (please add an additional page to highlight any gaps in your training appropriate to your stage - if there are no gaps, please confirm);

2. Logbook reports;

3. Record of study leave used in the last training year, and plans for the coming year;

4. Summary of your completed audit project, including details of where it was presented;

5. Completed audit assessment;

6. Completed MSF (on e-portfolio);

7. Evidence of WPBAs (minimum of 20, spread throughout the training year with approximately ten of each type);

8. Reflective Practice (minimum of 4);

9. Teaching Assessment (minimum of 2: one formal teaching session and one other, e.g. journal club);

10. Up-to-date copy of your CV;

11. Completed initial and up-to-date assessment forms;

12. MDTA (2 per year in years 2-5);

13. Evidence of completion of mandatory training as required by NHS Board.

 

Combined Infection Training (CIT)

1. WBAs: Trainees will be expected to undertake workplace-based assessment throughout the entire duration of their training in medical microbiology.

2. Assessment in CIT: Trainees will have to undertake at least 6 per year from the following:

  • Case-based discussion (CbD)
  • Evaluation of Clinical/Management Events (ECE)
  • Acute Care Assessment Tool (ACAT) - should be completed on clinical placements as unable to complete during lab based placements.
  • Mini-Clinical Evaluation Exercise (mini-CEX) - should be completed on clinical placements as unable to complete during lab based placements.

3. Undertake a minimum of 6 laboratory Directly Observed Procedural Skills (DOPS) as required during their medical microbiology and virology six-month training period (3 in each specialty).

4. Trainees are also required to undertake one Multi-source feedback (MSF) in CIT.

5. Also required are 4 multiple consultant reports per year.

Diagnostic Neuropathology

Please review pp 140-142 of the Diagnostic Neuropathology curriculum and ensure that you have completed the requirements for your training year.

 

Forensic Histopathology

Please note trainees should use the LEPT system.

  • ST1 = 6 CbDs, 6 DOPs, 4 ECEs, 1 MSF
  • ST2 = 6 CbDs, 6 DOPs, 4 ECEs
  • ST3 = 6 CbDs, 4 DOPs, 6 ECEs, 1 MSF
  • ST4 = 6 CbDs, 4 DOPs, 6 ECEs

Paediatric Pathology

1. Trainees should use the RCPath LEPT system for their training portfolio, MSF and WBA/SLEs.

2. For presentation and review at the ARCP trainees should submit the following:

  • Surgical pathology (including cytology cases) logbook (minimum of 1000 cases per year). Reflective notes from 20 cases per year should be presented for the ARCP.
  • Postmortem logbook (minimum of 100 cases per year). Reflective notes from 20 cases per year should be presented for the ARCP.
  • Educational Supervisors Structured Report (ESSR)
  • Workplace based assessments / Supervised Learning event logbook (LEPT)
  • Multi-source feedback during ST3
  • ARCP training summary sheet
  • Copies of completed audits(1 audit per year)

Royal College of Pathologists: Curriculum and ARCP information

 

Below are the evidence requirements for the following medical specialties:

ACUTE MEDICINE, CARDIOLOGY, CLINICAL GENETICS, CLINICAL NEUROPHYSIOLOGY, CLINICAL ONCOLOGY*, CLINICAL PHARMACOLOGY & THERAPEUTICS, CORE MEDICAL TRAINING, DERMATOLOGY, DIABETES & ENDOCRINOLOGY, GASTROENTEROLOGY, GUM, GERIATRIC MEDICINE, GIM, HAEMATOLOGY, INFECTIOUS DISEASES, MEDICAL ONCOLOGY, MEDICAL OPHTHALMOLOGY, NEUROLOGY, PALLIATIVE MEDICINE. RENAL MEDICINE, REHABILITATION MEDICINE, RESPIRATORY MEDICINE, RHEUMATOLOGY.

1. Please refer to the latest ARCP Decision Aids, published on the JRCPTB website by programme.  Dual accrediting trainees should check both specialty and GIM. *For Clinical Oncology, please see the decision aid available on the trainee/trainer section of the RCR website.

2. Dual accrediting trainees should complete the Summary of Training Calculation from the JRCPTB website which is designed to allow trainees to calculate their Acute Medical Take and Outpatient experience in a manner consistent with the GIM Decision Aid.

3. Please complete the Completion Date Calculator from the JRCPTB website and upload it to your personal library on e-portfolio.  It is your responsibility to make sure that this calculator is kept up to date and advise the ARCP panel of any amendments prior to the ARCP.

4. Please upload an up-to-date CV into your personal library space on e-portfolio.

5. If you have spent any time of the last year OOP (research), please upload your research supervisor's formal report to your personal library on e-portfolio.

Child and Adolescent Psychiatry

Full details of required evidence and guidance is available via this link.  In summary:

1. Please consult the C&AP Curriculum from page 184 for specific details of your requirements for ARCP and the information contained in the above link.  If you have any questions, please contact your TPD.

2. Please ensure that your evidence includes at least 6 episodes of reflective practice.

3. Please submit two Level 1 Supervisor reports – if in 2 posts in the ARCP year then one for each post.  If in 1 post for ARCP year then the first around the 6 months mark and the other before ARCP to allow completion of the level 2 report.

4. Please submit Level 2 trainer report. This is the end of year report completed by your Educational Supervisor/LPL

5. Please upload an up- to-date CV into your personal library space on e-portfolio.

 

Core Psychiatry

1. Please consult the Core Curriculum from page 75 for specific details of your requirements for ARCP. If you have any questions, please contact your TPD.

2. Please ensure that your evidence includes at least 6 episodes of reflective practice.

3. Please submit Level 1 Supervisor report for all posts being reviewed.

4. Please submit Level 2 trainer report. This is the end of year report completed by your Educational Supervisor.

Please note that if only six months are to be considered at the ARCP, then half the required numbers are needed.

 

Forensic Psychiatry

1. Please ensure that the following are submitted to your RCPsych portfolio:

  • a level 1 Clinical Supervisors Report (for each post being reviewed at ARCP);
  • a level 2 (Educational) Supervisor/TPD Report.

2. Please provide evidence of work place based assessments: the minimum number of each type of WPBA must be submitted as described within the Forensic Psychiatry curriculum.

3. Please provide at least 6 episodes of high quality reflective practice.

Medical Psychotherapy

1. Please consult the MEDICAL PSYCHOTHERAPY Advanced Psychiatric Training Competencies Work Place Based Assessment Guide.  Please see appendix 1, P15-18 for details. Please also refer to the explanatory notes in the Handbook for the Scottish National Training in Medical Psychotherapy (available from your TPD). If you have any questions, please contact your TPD.

2. Please ensure your evidence includes at least 6 episodes of reflective practice.

3. Please submit 2 x level 1 Supervisor's reports (one at approx. 6 months through the year, and one immediately prior to ARCP).

4. Please submit a level 2 trainer report. This is the end of year report completed by your Educational Supervisor.

5. Please upload an up to date CV into your personal library space on e-portfolio. 

 

Old Age Psychiatry

1. Please consult the OAP Curriculum from page 50 for specific details of your requirements for ARCP.  If you have any questions, please contact your TPD.

2. Please ensure that your evidence includes at least 6 episodes of reflective practice.

3. Please submit Level 1 Supervisor report for all posts being reviewed.

4. Please submit Level 2 trainer report. This is the end of year report completed by your Educational Supervisor.

5. Please upload an up- to-date CV into your personal library space on e-portfolio.

For all Surgical specialties (unless listed specifically below):

1. It is important that the Learning Agreement between Trainee and Educational Supervisor be completed on the ISCP website as early as possible after the start of your placement and that your Final Meeting be signed off by you and your supervisor in advance of your ARCP submission date.

2. Please update your ISCP portfolio with:

  • Completed Learning Agreement including the Interim Meeting and AES Final Report.
  • Clinical Supervisor’s report (from a Consultant who is NOT your AES). This needs to be completed before your AES Final Report as it is required for the completion of that Final Report.
  • Workplace-based assessments (WBAs) – a sufficient number of validated WBAs, as set in your Global Objectives, showing an appropriate spread of type (CBD, CEX, PBA, MSF as required) and performance level for your stage of training. These assessments SHOULD NOT be concentrated in the final part of your placement but SHOULD be completed throughout the duration of the placement.
  • List of completed Topics, matched to your WBAs.
  • Satisfactory up-to-date and validated logbook of operative procedures including Elective and Emergency sections.
  • Current CV.
  • Updated “Evidence” section : eg. audit, research, publications, courses, meetings attended, including any mandatory requirements set out in your Global Objectives.  This can include relevant historical evidence as long as this is dated and/or up-to-date (e.g. course certification).

3. Your portfolio evidence should also include a summary presentation of your progress towards the Checklist (benchmark, waypoint) appropriate to your specialty and stage of training. Checklists for each specialty.

General Surgery

It is extremely important that the Learning Agreement between Trainee and Educational Supervisor is completed on the ISCP website as soon as possible and your Learning Agreement Final Report is signed off by you and your supervisor ahead of your ARCP.

ISCP should be updated with:

  • Complete and satisfactory Learning Agreement; including AES Final Report
  • Clinical Supervisor’s report (from a consultant who is not your AES)
  • Satisfactory log book, completed Topics
  • Sufficient WPAs (13 CBDs, 13 CEXs, 13 PBAs and a miniPAT) 
  • CV uploaded onto your ISCP portfolio and an updated “evidence” section of your ISCP portfolio 
  • All colorectal and upper GI trainees: JAG summary sheet pdf to be saved to evidence section of ISCP portfolio.
  • ST3 and ST4: JCST ST4 trainee checklist to be saved to evidence section of ISCP portfolio.
  • ST5 and ST6: JCST ST6 trainee checklist to be saved to evidence section of ISCP portfolio.
  • ST7 and ST8: JCST CCT trainee checklist to be saved to evidence section of ISCP portfolio.

Ophthalmology

1. It is important that the Initial Appraisal meeting between trainee and educational supervisor take place and be documented in your portfolio as early as possible after the start of your placement and that your Final Appraisal Meeting is signed off in advance of your ARCP submission.

2. Please ensure that your training portfolio is up-to-date and that it complies with all the specifications laid out by the Royal College of Ophthalmologists.  Requirements for each stage of your training.

3. Please ensure that your assessments are spread over the training year rather than concentrated in a short period and that all assessments in your portfolio have been validated.

4. You should also ensure that you have completed any other requirements specified by your TPD.

 

Otolaryngology

Please provide the following:

1. Updated CV (please upload onto ISCP portfolio)

2. Emergency Logbook

3. WBAs (minimum of 40, including 1 MSF per year)

4. Checklists:

ST4: ST4 Emergency safe checklist should be saved to evidence section of ISCP portfolio.

ST6: ST6 checklist and ST6 Emergency safe checklist should be saved to evidence section of ISCP portfolio.

ST8: CCT trainee checklist to be saved to evidence section of ISCP portfolio.

Paediatric Surgery

All evidence must be submitted at least 2 weeks before the ARCP portfolio review date

1. Please check JCST guidelines for award of CCT in Paediatric surgery

2. Ensure AES and CS meetings (start and finish) completed and in place, which cover all attachments and demonstrating continuing progress towards attaining CCT

3. Ensure up to date CV, including all research and publications and relevant courses/conferences attended

4. Ensure validated up to date e-logbook, with indicative numbers and competency levels attained (ST6-8)

5. Ensure valid APLS

6. Annual MSF

7. Upload completed annual audit(evidence of 1 loop required )

8. Ensure you have completed self certification form if you have had days off sick since your last ARCP

9. Ensure you have 42 WBAs completed and validated (Minimum 26 PBAs, 4 CEX, 12 CBD ) Aim to spread these evenly throughout the year

10. Teaching attendance record (aiming for 70%)

11. Ensure GMC and Scottish trainee surveys complete

12. Other required courses for CCT include, Training the trainer, healthcare management, Good clinical practice, APLS

 

Plastic Surgery

Please confirm/provide the following:

1. 40 completed and fully validated WBAs per year including a minimum of 10 case based discussions, 10 procedure based assessments, 10 CEX and 1 miniPAT.

2. 75% attendance at pan-Scotland teaching events.

3. Fully completed and uploaded clinical supervisors and educational supervisors reports covering all attachments and demonstrating continuing progress towards attaining CCT.

4. Evidence of continuing medical education in plastic surgery by attendance at relevant courses and conferences.

5. Evidence of participation in a research and audit with 6 peer reviewed papers or podium presentations during the 6 year programme.

6. A fully completed and up to date logbook with a bar graph summarising progression towards indicative logbook numbers.

7. A fully completed and uploaded CV.

8. Trainees in their final year should complete a CCT checklist and add this as part of their evidence on ISCP.

 

Trauma & Orthopaedics

[All evidence must be submitted at least 2 weeks before the ARCP portfolio review date.]

1. Please check JCST certification guidelines on their website​ and complete the checklist if you are end of ST4, ST6 or ST7 (penultimate year checklist).

2. Ensure validated up to date e-logbook.

3. Ensure valid ATLS.

4. Please upload a fully updated CV to ISCP including all publications and research.

5. Email one power point slide with minimum indicative logbook procedures on it (only ST3-8 are counted).

6. Email one power point slide with courses or conferences attended since last ARCP.

7. Ensure annual MSF is up to date (ask your AES to suggest half of the raters and you the rest).

8. Ensure you have uploaded your completed annual audit.

9. Ensure you have completed the self certification form if you have had days off sick since your last ARCP.

10. Ensure fully completed and uploaded clinical supervisors and educational supervisors reports covering all attachments and demonstrating continuing progress towards attaining CCT.

11. Ensure you have 40 WBAs completed and validated (Usually 20 PBAs, 10 CEX, 10 CBDs but in earlt ST years DOPS may be used instead of some PBAs). If in LTFTT then this will be pro-rata.​​

 

Vascular Surgery

In addition to the generic requirements for ARCP for all surgical specialties, please provide the following:

1. Updated CV (please upload onto ISCP portfolio)

3. WBAs (minimum of 40, including 1 MSF per year)

4. Checklists relevant to stage of training:

5. Evidence of completion of on-line radiology modules by end of ST4.

Obstetrics & Gynaecology

Please submit/complete the following:

1. Educational Supervisor’s Report;

2. TO2 Form;

3. Regional Teaching Sessions Form;

4. ATSM Forms (ST6/7 only);

5. You are also required to follow the guidelines on the 2017-2018 training matrix and ensure you have completed all the requirements.

 

Paediatrics

Please consult:

Paediatric Cardiology

Please check the College's ARCP decision aid and ensure that you have fulfilled all requirements for your training level.

 

Paediatric Surgery

All evidence listed below must be uploaded to ISCP:

4. Educational Supervisor's structured report for the first learning agreement, and interim meeting information with comments from clinical supervisors for the second;

9. Summary of total operative experience to date (please add to CCT Progress Summary spreadsheet).  This must have neonatal experience clearly separated and identified.

General Practice

Please submit the required information as found in the links below:

3. If you are a GPST3, please consult Final ARCP Checklist.  Checking and completing this list will prevent you from getting an outcome 5 for missing evidence.

 

Public Health

Please submit to e-portfolio:

1. Requirements as listed on Faculty of Public Health website.

2. If you are OOP (Research): report from your research supervisor.

 

Occupational Medicine

Please submit (on paper):

1. Educational Supervisor's report.

2. Clinical Supervisor's report.

F1

A foundation doctor can only progress from F1 to F2 if they receive a satisfactory outcome at their ARCP which is held toward the end of the foundation year (June/July). The evidence to determine your progress is recorded in the e-portfolio and it is your responsibility to ensure that it is completed. The e-portfolio also provides an update of your progress.

The requirements for sign-off and a satisfactory ARCP (outcome 1) are as follows:

  • 100% coverage of the curriculum - this will be through SLE's, attendance at teaching, e-learning modules, reflective practice etc.  It is your responsibility to link the relevant learning to the relevant parts of the curriculum and ensure the coverage is recorded in your e-portfolio. It is expected that you will have at least 3 items of evidence for each capability and that a diverse range of evidence is used to gain 100% coverage of the curriculum. A minimum of one SLE per block is also required and it is expected that the range of SLE’s will be covered – i.e. you must have at least one CbD, one DOPS and one Mini-Cex in the year.  There is no maximum to the number of SLEs you can complete.
  • You should not be using an SLE and/or key output e.g. teaching session to link to more than 3/5 outcomes and the pop up on the portfolio reminds you of this.  It is important to be aware that if the ARCP panel consider that you have linked too many outcomes to the one assessment/output, you will be given an outcome 5 and asked to provide more evidence for proof of coverage of the curriculum by doing further work.
  • Developing the Clinical Teacher is a mandatory SLE and one must be completed in the year.
  • End of Placement Review forms from both the Clinical and Educational Supervisor for all placements of which two MUST be satisfactory including the final form.  If your Educational & Clinical Supervisor is the same person, then one Combined report is acceptable.
  • 2 complete and satisfactory TABs, from a suitable variety of assessors including at least 2 consultants or equivalent for each TAB.
  • All 15 GMC core procedures completed.
  • Current ILS certificate – this should be uploaded and confirmed on your Portfolio
  • PSA certificate indicating that you have passed – this should be uploaded and confirmed on your Portfolio
  • Evidence of participation in in systems of QA/QI.  This could be done by providing feedback on your posts through the NTS survey and/or Scottish Trainee Survey and record this under “Achievements" using the completion code or other relevant information.
  • Engagement in learning – 30 hours teaching attendance over the year.  It is appreciated that some of you may have difficulty reaching the 30 hour level due to rotas. Therefore the school has provided some additional guidance which can be viewed here.

Please note that just having three satisfactory End of Placement reviews will not mean that you will be automatically allowed to progress.  All other requirements must also be met.    

If all the required evidence is not available, and the panel consider that they cannot make a decision, you will be given an outcome 5 at your ARCP.  You will then be given a period of time (around 10 days) to provide all the relevant missing evidence and a repeat ARCP will be held.  If all the evidence is then present and satisfactory, you will be given an outcome 1 to progress.  However, it is important to note that the outcome 5 will remain on your e-portfolio record and this extra time may delay your registration with the GMC.  If you do not meet the requirement in the time frame, you will not be able to progress to your FY2 year which will have major implications.

If there are concerns about your competence and practice, you may be requested to attend an ARCP in person.

All trainees will have an ARCP even if they are not expecting an outcome 1, for example trainees on maternity leave or in less than full time training.

If you think that there may be a problem in relation to the completion of the evidence in your e-portfolio, please get in contact with your Foundation Programme Director as soon as possible.  If requirements are not met, the Scottish Foundation School will not be able to sign you off.  This will mean that you will have to continue to work as a FY1 for a further period of time. This may then prejudice your sign off as FY2 and consequently your ability to get a ST post.  This is a very important time for you.  Please ensure that you meet all the requirements.

Please note that if you have been absent from work for more 20 working days (apart from A/L)  but including Sick Leave, Paternity Leave, Special Leave etc you will be required to do an extra 4 month block.  This is due the GMC regulations where sign off is both related to competency and time.

F2

All trainee doctors are required to have a ARCP every year to determine their progress as a FY2, this will take place in June/July and if satisfactory, an outcome 6 will be awarded i.e. completion of programme. This will result in a FPCC being awarded which is required for progression into specialty training.

  • Engagement in learning – 30 hours teaching attendance over the year.  It is appreciated that some of you may have difficulty reaching the 30 hour level due to rotas. Therefore the school has provided some additional guidance which can be viewed here.
  • Evidence that you can manage, analyse and present at least one quality improvement project and use the results to improve patient care.
  • Completed SOAR declaration.

Please note that just having three satisfactory End of Placement reviews will not mean that you will be automatically signed off.  All the other requirements must be met for you to be signed off. 

If all the required evidence is not available, you will be given an outcome 5 at your ARCP which will require you to provide all the relevant evidence within 10 days when a repeat ARCP will be held.  If all the evidence is then present, you will be given an outcome 6 as you will have completed the programme. However, it is important to note that the outcome 5 will remain on your Portfolio record. If you do not meet the requirement in the time frame, you may not be able to progress to Specialty Training which could have major implications.  If there are concerns about your competence and practice, you may be requested to attend an ARCP in person where options for future completion of foundation training can be discussed.

As a fully registered doctor, you are also required to complete a form as part of your revalidation.  To make your declaration as part of the process, a Scotland-wide IT package has been mandated called SOAR (Scottish Online Appraisal Resource).  Information about this has already been sent to you and the declaration initiated.  It is your responsibility to complete it so that it can be countersigned.  If the form is not signed by all parties by the time of the ARCP, this will result in an outcome 5. 

It is your responsibility to sign the ARCP form in Portfolio after it has been completed. 

If you are awarded an outcome 6, you will be sent a link in your Portfolio which will require you to complete the Destination Survey.  Completion of the Destination Survey will release your FPCC form into your Portfolio.  This form has been signed by the Foundation School Director to confirm that you have satisfactorily completed the Foundation Programme.  Once this has been completed, I would suggest you download it from your Portfolio and keep copies as they will be required in the future.  The School will not provide hard copies for you this year or in the future.

Please note that all trainees are given an ARCP.  I am aware that for some of you, especially those on maternity leave or working less than full time, an outcome 6 may not be the outcome at this time as you still have further time in the Foundation Programme.

If you think that there may be a problem in relation to the completion of the evidence in your Portfolio, please get in contact with your Foundation Programme Director as soon as possible.  If requirements are not met, the Scottish Foundation School will not be able to sign you off.
